Full Job Description:
POSITION SUMMARY
A well-organized and highly motivated individual that can obtain and analyze information in a collaborative method as it pertains to the needs of the loved ones and family members of a current or former patient in our care. These efforts are including but not limited to, planning, referrals, education, resources, and integration of care from a family/loved one perspective. Family care coordination is a collaborative process to meet a family’s and /or loved one’s comprehensive needs through communication, support and connection to available resources that promote quality care. They will also oversee family engagement through collaboration with clinical, communications and quality assurance staff. Excellent customer service skills and problem solving are vital to this position.
QUALIFICATIONS
Proficiency in the skill of reading and writing and communication are essential, as well as one’s ability to work in a collaborative atmosphere with colleagues, business professionals, families, and patients. Knowledge of coaching, assessment methods, support group techniques, facilitation of psychoeducational presentations, as well as knowledge of residential treatment programs is required. An effective Family Care Coordinator will have an understanding of human behavior and motivational interviewing to assist in facilitating education and providing support for family members. A comprehensive knowledge of Microsoft Office programs and the EMR is essential for the production of appropriate documentation to provide necessary information as it pertains to tailoring resources to satisfy the needs of the family. Time management and organization skills will be considered professional strengths of the Family Care Coordinator for the purpose of completing their responsibilities and engaging the family efficiently. The initial assessment of these qualifications will be conducted during pre-hire interview by senior management staff members.
A Bachelor’s Degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in the human services) or other related field and I year of experience in a health or human services agency, preferably in a drug and alcohol setting. If a person’s experience did not take place in a drug and alcohol setting, the individual’s written training plan will specifically address a plan to achieve coaching competency in chemical dependency issues.
OR
An Associate’s Degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing with 2 years of experience in a health or human service agency, preferably in a drug and alcohol setting. If a person’s experience did not take place in a drug and alcohol setting, the individual’s written training plan will specifically address a plan to achieve coaching competency in chemical dependency issues.

Pay: Up to $22.00 per hour
