Behavior Services Peer Support Specialist (New Haven)

Assist Employment Case Manager in providing support to a caseload of individuals requesting employment support services within the clinical team to which assigned.
Assist Employment Case Manager in maintaining comprehensive vocational case records for individuals on caseload.
Conduct job clubs and peer support groups and activities.
Assist in duties assigned by Case Management for carrying out plan for establishing a Resource Center, job club, public education plan and relationships with business resources in the area. This will include participation in the development of television shows, presentation, classes, public service announcements, etc. as well as researching business information.
Educate other team members on employment principles to facilitate an integrated approach to employment outcomes.
Assist Employment Case Managers in conducting employment related assessment activities to generate recommendations for job placement.
Assist in the implementation of recovery plans to assure attention to dimensions of recovery, cultural competence and evidence-based practices.
Coordinate time off with prior approval from Director of Behavioral Health.
Assure that services are provided in accordance with evidence-based practices for supported employment, recovery, dual diagnosis, engagement, etc. as well as promising practices with regard to Peer Mentoring.
Facilitate, promote and ensure community integration and the realization of valued social roles.
Demonstrate and Implement incidental teaching.
Facilitate teamwork in implementing individual recovery and employment goals.
Prepare monthly reports for review by Lead Case Management and for case conferences.
Maintain a safe and clean work environment and assures that agency property is well maintained.
Follow the Marrakech procedures, protocols, and Policies and Procedures manuals as well as conform to the work culture and expectation of the CMHC
Assure that program data is collected as determined by individual and overall program requirements.
Attend staff meetings and required in-service training.
If applicable, operate motor vehicles safely and in accordance with State Law (agency and personal automobiles) while on work related duties as appropriate.
Report unsafe conditions and situations immediately per agency policy and procedures
Represent the values, mission and vision of the organization
Serve all customers with alacrity
Communicate with co-workers and other providers accurately and professionally.

QUALIFICATIONS:

  • High School diploma and 5 years of related experience with young adults with dual diagnosis including Intellectual Disabilities & Mental Health
  • Must be 21 years old or older
  • Certification in CPR, DDS Medication Certification & PMT preferred.

PHYSICAL DEMANDS/WORK ENVIRONMENT

  • Must be physically able to perform the duties of the position which may require talking, hearing, lifting unspecified weights, standing, bending, kneeling, stooping, crouching, stretching, crawling, pushing and/or pulling.
  • There are multiple stories of stairs that may have to be walked up & down frequently to work with individuals on different floors.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Knowledge of, implements and monitors Behavioral Support Plans, Behavioral Guidelines and/or crisis plans for program individuals.
  • Knows and follows eating and/or food Guidelines for all individuals, as appropriate
  • Assists individuals with personal care and hygiene needs.
  • Utilizes approved behavior management techniques to improve and enhance an individual’s ability to be included in the community and/or social situations.
  • Provides required communication and documentation to meet program standards. Completes all required paperwork using web based systems and paper documentation.
  • Acts as Advocate for individuals, helping them to make choices in their day and community environment. Acts on their behalf, as an independent voice for the person, asking questions and ensuring the person's wishes, values and beliefs are considered when any decisions are made.
  • Works with Program Manager to design and implement a plan of activities that meets the assigned individuals’ needs, interests and IP goals. Ensures that the activity schedule is implemented as planned.
