ACT hiring medication adherence nurse

AIDS Connecticut currently has a job opening for:

Job Title: Medication Adherence Nurse

Reports To: Director of Treatment and Care

Goal: To provide medication adherence services to HIV-positive individuals, including monitoring and assisting with adherence to medication regimens in a community-based setting.

Functions/Objectives:
  • Provide medication adherence assistance to HIV-positive clients
  • Conduct initial and ongoing assessment of clients, including level of medication adherence, motivation and barriers to adherence according to Department of Public Health and AIDS Connecticut protocols
  • Maintain direct contact with client's care team to report successes and challenges in medication adherence and other health outcomes
  • Collect and input data into electronic data collection software (CAREWare)
  • Identify and acknowledge electronic referrals/requests for services in CAREWare
  • Develop care plan with client and/or other service provider to address barriers identified via assessment
  • Monitor care plan and outcomes in client record on an ongoing basis
  • Collaborate with network of providers involved in client care
  • Provide health education and referrals as needed
  • Facilitate adherence support groups
  • Complete program reports as necessary
  • Other duties as necessary
Qualifications:
  • Current Connecticut LPN or RN License
  • Comfort working with diverse clientele and staff; knowledge and experience with HIV/AIDS and HIV/AIDS clients preferred
  • Bilingual (English/Spanish) a plus
  • Strong organization and time management skills required
  • Excellent oral and written communication skills and ability to effectively communicate with diverse clientele, including HIV-positive clients
  • Experience working in a fast pace environment and ability to respond to change quickly
  • Creativity, flexibility and ability to work as part of a team with excellent time management skills
  • Ability to work with limited supervision (face to face and/or virtual) is expected
  • Ability to flex time to meet clients' needs
  • Knowledge of CAREWare or any other electronic record system is preferred
  • Knowledge of community resources and networks of HIV care preferred
  • Must have a valid driver's license in good standing and ability to travel in the greater Hartford Area
This is a non-exempt full-time position based in Manchester/Hartford Areas. The position's salary range is based on experience, with a competitive benefits package.
