Perfect Care is seeking dynamic and reliable Direct Support Staff that provides necessary instructions to clients so as to assist them with maintaining their independence and safety in the community.

Direct Support Professionals

• Ensuring safety and well-being of each patient in alignment with the care plan.
• Promoting development of a healthy lifestyle to meet health and wellness objectives.
• Documenting entries on log sheets and calendars to accurately report patient daily progress.
• Coaching and Assisting clients with Activities of Daily Living Skills to maintain a clean and orderly environment. (Meal planning and preparation, shopping, housekeeping, transportation to appointments (e.g., changing linens, washing dishes, vacuuming/dusting, laundry, mending clothing repairs), basic household tasks (e.g., regulating home temperature, storing food appropriately, resolving issues about bill paying).
• Ability to develop and maintain cooperative, professional relationships with clients, other staff and community support.

Direct Support Professional Qualifications

High School Diploma/GED required.

Must Possess a valid Connecticut driver’s license and reliable form of transportation.

RA/PCA/CNA a plus but not required.

Job Types: Full-time, Part-time

Pay: $15.00 - $17.00 per hour
