CAN Outreach Specialist

Columbus House opened its doors in 1982 to provide services for
men and women at least 18 years of age. Our goals quickly broadened from
the mere provision of food and shelter and simple survival, to
understanding and working toward overcoming the problems which cause
people to become homeless.

serve people experiencing homelessness or at imminent risk by providing
life-saving outreach, shelter and housing and by fostering their
personal growth and independence. We advocate for and create affordable
housing to end homelessness in our communities."
Purpose
Under the dual direction of the Director of Emergency Services and
CAN Manager, the CAN Outreach Specialist supports the mission of the
Greater New Haven Coordinated Access Network Task Force members, public
and private partners, and United Way team to develop, implement, and
measure homeless services that address homelessness for chronically
homeless adults, youth and families. The CAN Outreach Specialist will
work closely with the CAN Manager, CAN System Coordinators, Family
Navigators, and community providers to assist individuals access
emergency shelters, and exit homelessness to a variety of housing
options in the GNH CAN service area (Ansonia, Beacon Falls, Bethany,
Branford, Derby, East Haven, Guilford, Hamden, Madison, Milford, New
Haven, North Branford, North Haven, Orange, Oxford, Shelton, Seymour,
West Haven, or Woodbridge).
Duties and Responsibilities
- Verification Support
- Connect with clients and other social service providers in order to secure verification of homelessness
- Work with case managers of the client to assess their housing history
- Provide linkages for clients who are in need of social, mental health, educational, vocational, recreational, spiritual, HIV and and/or substance abuse services
- Coordinate and collaborate with other agencies and social service providers
- Engage in “street” outreach in all kinds of weather conditions
- Attend weekly and monthly agency, program and network meetings
- Monitor all cases assigned and complete all documentation/charting required by the GNH CAN
- Complete First Contact Forms
- Enter and maintain all client charts accurately and timely into the Homeless Management Information System database Case Worthy
- Other related duties as assigned
Qualifications
- High School graduate or GED
- Excellent organizational and time management skills with a deadline-oriented mindset
- Ability to analyze and exercise sound judgment
- Ability to work with electronic databases i.e. Smart sheets, HMIS
- Strong written and oral communication skills employing the English language
- Able to function as part of a multi-disciplinary team
- Familiar with mental health, substance abuse and dual diagnosis issues
- Experience providing person-centered/strength-based services to vulnerable populations
- Ability to relate to people of different economic and ethnic backgrounds
- Able to meet all documentation requirements
- Knowledgeable about the challenges and barriers that homeless people face
- Valid drivers’ license with a good driving history
Supervised by: Director of Emergency Services and CAN Manager
Schedule: Sunday & Monday 8:30am-4:00pm, Wednesday 9am- 5pm,Tuesday & Thursday 12pm- 8pm
Salary range: $19.23 - $21.23
